The Northwest Community Health Innovation Lab is an initiative of the Northwest Regional Primary Care Association (NWRPCA) that brought together six community health centers from Alaska, Oregon, and Washington to explore innovative approaches to community health. The project utilized a vital conditions framework to drive systemic changes promoting health equity and community well-being.
